Admissions Application Form

Thank you for your interest in Pathways Centre for Education School. Please note that the following items are required to complete an application:

- $100 non-refundable application fee (please e-transfer to accounts@pathwayscfe.com before completing the following form)

- Current photo of the applicant

- Copy of the applicant's birth certificate or passport

- Most recent report card or progress report

- Final June report card from the two previous academic years

- Psychoeducational/Psychological assessments, if applicable

- IEP, if applicable
